Interpretation:

Add up your total score out of 50.

  • 41–50: Established Practice – Your responses indicate that several effective feedback practices are already part of your leadership approach. Identify the lowest-rated item and use it as your next development priority.

  • 31–40: Developing Practice – You have a foundation in place, but your feedback habits may not yet be consistent. Focus on one or two practices that would improve the quality and regularity of your conversations.

  • 21–30: Inconsistent Practice – Feedback may depend too heavily on the situation or immediate need. Select one preparation practice and one follow-through practice to use consistently.

  • 10–20: Limited Practice – Begin by establishing a repeatable structure for preparing, conducting, and following up on feedback conversations.

The RA-RA Feedback Form

A structured process for preparing, conducting, and following through on feedback

The RA-RA Feedback Form helps leaders and team members approach feedback as a continuing process rather than an isolated conversation. Its four stages provide a consistent structure for establishing readiness, aligning expectations, reflecting on performance, and agreeing on adjustments.

The form creates a shared record of expectations, observations, and next steps. Used consistently, it can improve the continuity of performance conversations and help both participants remain accountable for progress.

Use the form during scheduled feedback sessions, developmental conversations, project reviews, or other discussions in which clear expectations and follow-through matter.
